These Terms of Service ("Terms") govern your use of reviveteeth.com and your purchase of Revive Veneers from Revive Teeth LLC ("Revive Teeth," "we," "us"). By using the site or placing an order, you agree to these Terms, our Privacy Policy, our Shipping Policy, our Disclaimer and Important Safety Information, and our Refund, Remake & Returns Policy, all of which are incorporated here by reference. If you do not agree, do not use the site or place an order.
Revive Teeth LLC is a Wyoming limited liability company. We sell directly to consumers in the United States. You must be at least 18 years old to use the site or place an order. By ordering, you confirm you are 18 or older.
Revive Veneers are custom-made using the impressions you provide. Production begins once we receive your impressions, and because each set is manufactured uniquely for you, your order cannot be cancelled for a change of mind once production has begun. Cancellations, remakes, and the limited refunds available are governed entirely by our Refund, Remake & Returns Policy, which forms part of these Terms.
Revive Teeth LLC is a cosmetic product company, not a dental practice. We do not provide dental services, dental treatment, or clinical care of any kind. No dentist-patient relationship is created by your use of this site or your purchase of any product.
Revive Veneers are a cosmetic product only. They are not a treatment for any dental or medical condition, and they do not diagnose, treat, cure, or prevent any disease or dental problem.
Dr. Brandon Turley, DDS is the designer of Revive Veneers and its 7-Point Smile Design Protocol. He is not a treating dentist for any Revive Teeth customer, he does not examine, diagnose, or treat Revive Teeth customers, and he does not provide them with dental care or advice. Your purchase of Revive Veneers does not constitute a dentist-patient relationship of any kind.
Revive Veneers are not a substitute for professional dental care. You are responsible for your own oral health, and you should see a licensed dentist for any dental or medical concerns, including whether using a removable oral appliance is safe for you. By purchasing, you accept that you use the product at your own discretion and assume the ordinary risks of wearing a removable cosmetic oral appliance, and you agree to stop using it and consult a dentist if you experience pain, irritation, or any problem.
The site, the Smile Quiz, and all content we provide are general information about a cosmetic product. They are not dental or medical advice, are not tailored to your individual circumstances, and should not be relied on as a substitute for consulting your own dentist.
Prices are shown on the site and are subject to change. Payment is processed by Shopify Payments. When you purchase Revive Veneers, you authorize us to charge your selected payment method for your order, including applicable taxes and shipping. We may refuse, limit, or cancel any order (for example, for suspected fraud or pricing errors). If an order is canceled before production begins, we will refund amounts paid.
Production and delivery timeframes are estimates and are governed by our Shipping Policy. Because the product is custom-made, manufacturing takes time after we receive your impressions; we are not responsible for carrier delays outside our control.
All cancellations, remakes, and refunds are governed by our Refund, Remake & Returns Policy, which forms part of these Terms. Finished, custom-made veneers are not eligible for change-of-mind or satisfaction refunds. Fit and finish issues are resolved through remakes, and limited refunds apply only as described in the policy.
If you submit photos or other content (for example, for fit or remakes), you grant us permission to use those images as needed to fulfill and support your order. We will only use your photos for marketing or testimonials if you give separate written permission under our media release. You are responsible for the content you submit and confirm you have the right to share it.
The site, its content, the Revive Teeth and Revive Veneers names, logos, and Dr. Turley's 7-Point Smile Design Protocol are owned by or licensed to Revive Teeth, and are protected by law. We grant you a limited, personal, non-transferable license to use the site to learn about and purchase the product. You may not copy, reproduce, or exploit our content or marks without permission.
You agree not to misuse the site, interfere with its operation, attempt unauthorized access, scrape its content, or use the site for any unlawful purpose.
Except for the specific Fit Promise set out in our Refund, Remake & Returns Policy, the site and the product are provided "as is" and "as available." We disclaim all other warranties to the fullest extent permitted by law, including implied warranties of merchantability and fitness for a particular purpose. We do not warrant any specific cosmetic result, and results vary by individual.
To the fullest extent permitted by law, Revive Teeth's total liability arising from or related to the product or these Terms will not exceed the amount you paid for the product at issue, and we will not be liable for indirect, incidental, special, or consequential damages.
You agree to indemnify and hold harmless Revive Teeth and its members, employees, and partners from claims arising out of your misuse of the site, the product, or your breach of these Terms.
PLEASE READ THIS SECTION CAREFULLY. IT AFFECTS YOUR LEGAL RIGHTS, INCLUDING YOUR RIGHT TO FILE A LAWSUIT IN COURT AND YOUR RIGHT TO A JURY TRIAL.
You and Revive Teeth agree that any dispute, claim, or controversy arising out of or relating to these Terms, your use of the site, or your purchase of any Revive Teeth product (collectively, "Dispute") will be resolved exclusively through binding individual arbitration, except as provided below. This includes disputes about the formation, validity, enforceability, scope, or interpretation of this arbitration agreement.
Arbitration Administrator and Rules. Arbitration will be administered by the American Arbitration Association ("AAA") under its Consumer Arbitration Rules, which are available at www.adr.org. If the AAA is unavailable or declines to administer the arbitration, the parties will agree on an alternative administrator; if they cannot agree, a court will appoint one.
Individual Arbitration Only; Class Action Waiver. ALL DISPUTES MUST BE BROUGHT IN YOUR INDIVIDUAL CAPACITY AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS, COLLECTIVE, CONSOLIDATED, OR REPRESENTATIVE PROCEEDING. The arbitrator may not consolidate more than one person's claims and may not preside over any class or representative proceeding. If this class action waiver is found unenforceable in a particular case, the arbitration agreement will not apply to that case, and it must be brought in court.
Location and Format. Arbitration will be conducted in Wyoming or, at your election, by telephone, videoconference, or written submission. For claims under $10,000, the arbitration will be conducted solely based on documents submitted to the arbitrator, unless you request a hearing.
Fees. Filing fees and arbitrator fees will be allocated in accordance with the AAA Consumer Arbitration Rules. Revive Teeth will not seek attorneys' fees or costs in arbitration unless the arbitrator finds your claim was frivolous.
Exceptions. Either party may bring an individual claim in small claims court if it qualifies. Either party may also seek emergency injunctive or other equitable relief in a court of competent jurisdiction to prevent actual or threatened infringement, misappropriation, or violation of intellectual property rights.
Opt-Out Right. You may opt out of this arbitration agreement by sending written notice to support@reviveteeth.com within 30 days of first agreeing to these Terms. Your notice must include your name, the email address associated with your account, and a clear statement that you wish to opt out of arbitration. If you opt out, all disputes will be resolved in the courts specified in Section 16.
Survival. This arbitration agreement survives termination of these Terms and your relationship with Revive Teeth.
These Terms are governed by the laws of the State of Wyoming, without regard to its conflict-of-law rules. Any Dispute that is not subject to arbitration under Section 15, including where you have validly opted out or where the class action waiver has been found unenforceable, will be brought exclusively in the state or federal courts located in the State of Wyoming, and you and Revive Teeth consent to the personal jurisdiction of those courts. As noted in our Privacy Policy, this governs interpretation of these Terms and does not remove rights customers may have under the laws of the state where they live.
If any provision of these Terms is found invalid or unenforceable, that provision will be enforced to the maximum extent permissible and the remaining provisions will remain in full force and effect, except as expressly provided in Section 15 with respect to the class action waiver. These Terms, together with the policies incorporated by reference, constitute the entire agreement between you and Revive Teeth regarding the site and your purchase, and supersede any prior agreements on that subject.
We may update these Terms from time to time. The version in effect when you place an order applies to that order, and continued use of the site means you accept the current Terms.
